The Lewiston Civic Theatre now offers an online form for auditions.

Please note:
This form requires Adobe Reader© version 8.1 to view, fill out, and send via email. If you do not have Adobe Reader© version 8.1, you may download a free version at http://www.adobe.com/products/acrobat/readstep2.html.

Steps to fill out the form and send:

1. Once you have Adobe Reader© installed you may click on the form link below to open the form.
Note: If your computer is set to save the form file, you may do so by saving the file to a location on your local hard drive. If you have chosen to save the file to your hard drive, your next step will be to open the Adobe Reader© (when you download the current reader, a icon will be automatically created on your desktop), and then open the saved audition file.

2. After the file is open in Abobe Reader© you will set your mouse cursor on each field and then type in your information.

3. Once you have completed the form, please print a hard copy of the form for your records and then click on the "submit to LCT" button at the top right side of the form. This will bring up your email composer with the address and form attachment automatically set.

4. Click the send button on your email composer and your audition form will be emailed to the theatre for review.

You may also wish to save a copy of your completed form to your hard drive for future reference.
